Hearing Choice is a National Network designed for hearing healthcare providers and consumers. Hearing Choice is committed to improving the hearing health care system. To become a provider of Hearing Choice you must fill out the provider application in its entirety. After the application has been reviewed and approved (please allow 5 working days) you will be able to access the provider area. Each provider is required to offer to the member a maximum of 30% off your list price of durable goods and services. In addition your office must be in good standings with Hearing Choice, have a current valid license to dispense hearing aids and perform Audiological diagnostics, if an audiologist.

This does not apply to states that do not require dispensing licensure or states that do not require a dispensing licensure if audiology licensure is current and in good standings.
